Conditions for participation in the tender, required documents and criteria to be applied in qualification evaluation: 4.1. Conditions for participating in the tender and required documents: 4.1.1. Address declaration for notification, as well as telephone and, if available, fax number and e-mail address for contact, 4.1.2. Certificate from the Chamber of Commerce and/or Industry or Professional Chamber in which it is registered in accordance with the legislation; 4.1.2.1. In case of a real person, a document showing that he/she is registered with the Chamber of Commerce and/or Industry or the relevant Chamber of Professional Affairs, depending on his/her interest, obtained in the year of the first announcement or tender date, 4.1.2.2. In case of a legal entity, a document showing that the legal entity is registered in the registry, obtained from the Chamber of Commerce and/or Industry in which the legal entity is registered in accordance with the legislation, in the year of the first announcement or tender date, 4.1.2.3. Document/information required in accordance with the relevant legislation to continue the professional activity with the work subject to tender: BIDDERS SHALL HAVE ISO:9001 CERTIFICATE. 4.1.3. Signature declaration or signature circular showing that the person is authorized to bid; 4.1.3.1. In case of a real person, notarized signature declaration, 4.1.3.2. In case of a legal entity, the Trade Registry Gazette showing the latest status indicating the partners, members or founders of the legal entity and the officers in the management of the legal entity, depending on their relevance, or the documents certifying these issues and the notarized signature circular of the legal entity, 4.1.4. A written undertaking stating that the situations listed in paragraphs (a), (b), (c), (d), (e), (g) and (i) of Article 10 of Law No. 4734 are not present, 4.1.5. Offer letter, the form and content of which is determined in the Administrative Specifications, 4.1.6. Temporary guarantee, the form and content of which is determined in the Administrative Specification, 4.1.7. All or part of the procurement/work subject to tender cannot be outsourced to subcontractors. 4.1.8. Document proving the purchase of the tender document, 4.1.9. Declaration regarding private companies of which it is a partner and capital companies of which it owns more than half of the capital, depending on whether it is a real or legal person, 4.1.10. A letter of undertaking stating that if the work experience certificate submitted by the legal entity bidder belongs to a partner who owns more than half of the shares of the same legal entity, the work experience document to be submitted will not be made available to another legal entity. 4.2. Documents regarding economic and financial sufficiency and the criteria these documents must meet 4.2.1. Documents to be obtained from banks: Documents to be obtained from domestic or foreign banks showing the unused cash loan or unused letter of guarantee loan or free deposit at banks in an amount to be determined by the bidder, not less than 5% of the bid price. 4.3. Documents related to Vocational and Technical qualifications and the criteria that these documents must meet 4.3.1. Work experience documents: Documents showing the bidder's experience in the procurement subject to the tender or purchases of a similar nature to be determined in the tender document, which have been carried out in the country or abroad, in the public or private sector in the last five years and accepted by the administration as complete and free of defects. A work experience certificate for a single contract for the purchase subject to the tender or purchases of a similar nature to be determined in the tender document will be submitted at the rate of 25% of the price offered by the bidders, depending on the nature of the work subject to the tender. 4.4. As a similar job in this tender, ALL KINDS OF PLASTIC MATERIAL MANUFACTURING AND SALES 5. The most economically advantageous offer will be determined on the basis of the lowest price. 6. Only local bidders can participate in the tender. 7. The tender document can be viewed at the address where the tender will be held and can be obtained from the same address for 250 YTL / Million TL.
